Learn the skill
Work with an adult who can model letter sounds. Learn any new sounds first. Point to each letter from left to right, join the sounds, and read the whole word. After a change, check every letter again. An adult may read the directions, but let the learner try the words. Pointing or saying an answer is fine; no timer or recording is needed. Questions check letter changes, not spoken reading.
Worked example
Mat becomes sat when m changes to s. Sat becomes sap when the last t changes to p. Read every letter, not just the changed one.
Read together before the questions
mat, sat, sap, tap. Sam sat. Read the chain slowly, then read it again. Tell what the sentence or phrases mean. Your helper can model a sound and let you try again. This reading is not automatically scored.
