A report can become more accurate when it says what remains unknown. Missing outcomes are not automatically successes or failures.

Original fiction: The Incomplete Follow-Up

A fictional project invited 80 participants to try a planning tool. Forty completed a follow-up form, and 30 of those respondents reported using the tool again. The team did not learn whether nonrespondents continued using it. A draft headline says Most invited participants kept using the tool. A reviewer replaces it with Thirty of forty follow-up respondents reported reuse and notes that the other forty participants' outcomes are unknown.
